If reporting by email, it is strongly encouraged that the following information be included:

• Name(s) and contact information of the reporter(s), unless reporting anonymously. 
• Name(s) of the individual(s) alleged to have committed misconduct. 
• Type of misconduct alleged. 
• Approximate date(s) the misconduct was committed. 
• How the reporter(s) acquired the knowledge of the alleged misconduct. 
• Names of the other individuals who might have information regarding the alleged misconduct. 
• Summary statement of the reasons to believe misconduct has occurred.
